fix(core): keep the deferred-tool bridge halves on the same tool (#12539)

* fix(core): keep the deferred-tool bridge halves on the same tool

Two findings deferred from #10410 (#11321):

F1' - tool_search's select: and tool_call each resolved a model-supplied
name case-insensitively with "last match wins" over getAllToolNames().
ensureTool reorders that list when it moves a lazily built tool from the
factory map into the tool map, so the two halves could pick different
tools for a name that differs from two registered tools only by case.
Both halves, and tool_call's classifier projection, now share
resolveRegisteredToolName: an exact match wins, a single case-insensitive
match is used, and several case-insensitive matches are refused with the
candidates listed instead of guessed.

R16-1 - tool_call invoked any hidden deferred tool by name, so after the
tool's schema or MCP server changed it could run arguments written
against a declaration the model no longer holds. tool_search now records
a fingerprint (serialized declaration plus MCP server) of each hidden
tool it returns, and tool_call refuses a reviewed tool whose fingerprint
changed, asking for a fresh select:. A tool never reviewed in the session
still works by name, so an unchanged tool costs no extra round trip,
including after /compress.

Not run locally: vitest, typecheck and eslint are left to CI. The
resolution and refusal paths were exercised in a tsx process against the
same registry double the tests use.

* fix(core): resolve bridged names in the classifier transcript the same way

Review of #12539: classifier-transcript.ts still resolved a bridged
tool_call target with the last-match loop, so the historical-transcript
projection could pick a different tool than tool_call for a name that
differs from two registered tools only by case. It now uses
resolveRegisteredToolName, and a name matching several tools by case
projects name-only, as tool_call refuses it.

* fix(core): close four deferred-bridge review findings on #12539

R1-1 (Critical): deferredDeclarationFingerprint hashed the whole schema,
including the free-text description. WebSearchTool rebuilds that description
from the current month/year on every schema access (web-search.ts:997-1004 ->
916-921) and ReadFileTool rebuilds it from config.getEffectiveInputModalities()
(read-file.ts:628-637), both intentionally so a long-lived qwen serve/ACP
process is not stale. An unchanged tool's fingerprint therefore drifted across
a month boundary or a /model switch and the new tool_call gate refused a
legitimate call whose parametersJsonSchema was byte-identical. The fingerprint
now covers the invocation contract (server + schema.name +
parametersJsonSchema); the getters keep recomputing. The pinned "declaration
changed" refusal is retargeted to a parameter-contract change so it stays
meaningful.

R1-12: the select: parser deduped entries on stripped.toLowerCase(). Since
resolveRegisteredToolName makes two names differing only by case two genuinely
different tools, that silently dropped one spelling: it appeared in none of
missing/truncated/ambiguous, was never recorded as reviewed, and then reached
tool_call, which resolves it exactly and takes the never-reviewed pass-through
-- the failure #11321 exists to close, via the path the ambiguity message
recommended. Dedupe now keys on the resolved identity, so repeated spellings of
one tool still collapse while distinct case variants are both reviewed.

R1-14: the ambiguity remedy rendered "request the exact name: <rejected>",
putting the spelling that is by construction NOT registered into the
instruction slot; a model following it literally re-issued a byte-identical
select: that loop detection counts as a duplicate. The message now mirrors the
twin refusal at tool-call.ts:112 -- the rejected spelling is echoed in quotes
and the actionable slot holds select:-able candidates.

R1-4: pin the two handshakes the changed-declaration guard depends on. The
lookup must key on the resolved target.name (tool_search records under the
registered name while models invoke with spellings needing resolution), and
recordReviewedDeclaration must overwrite an existing entry so the
refusal -> re-review -> recovery loop actually closes. Adds a case-variant
refusal case and a real-ToolRegistry integration test that kills both mutants.

* fix(core): sync the deferred bridge halves on aliases, prune review records, update docs

R1-13: canonicalize the legacy alias before resolving in tool_search's
select: path — both in the dedupe key and in returnSchemas — so the
discovery half answers exactly like the invocation half. select:replace
now reviews edit instead of reporting "Not found: replace", and
select:replace, edit reviews one tool instead of two.

R1-10: reclaim a removed tool's reviewed declaration by replacing it with
a tombstone instead of retaining the serialized schema for the registry's
lifetime. The tombstone can never equal a live fingerprint, so a reconnect
that republishes an identical declaration still forces one re-review
rather than taking the never-reviewed pass-through. A tool that was never
reviewed keeps no entry, so removal cannot grow the map.

R1-8: pin the ambiguous -> name-only branch of
ToolCallTool.toAutoClassifierInput, the only branch of that projection
with no test of its own.

R1-3 / R1-2: bring docs/design/deferred-tool-call-bridge.md and its zh-CN
counterpart in line with the shipped behaviour — the shared name-resolution
rule, the declaration fingerprint and what it deliberately excludes, the
remaining presentation-mark gap, the server-name-label gap, the review
record's lifetime and tombstone, and the new refusal classes in both
Verification lists. Also records that a refusal resolving to no target
keeps the wrapper bucket for ACP parameter-error loop accounting.

* fix(core): tombstone the reviewed declaration on single-server re-discovery

Two correctness fixes for code this branch added in its latest round, both
verified red-first before the fix and pinned by a collocated test.

R4-1: `invalidateReviewedDeclaration` was wired into `removeDiscoveredTools`
and `removeMcpToolsByServer` but not into `discoverToolsForServer`, which
hand-inlines the same removal. That method is the route `/mcp reconnect`, the
MCP management dialogs and `DiscoveredMCPTool.attemptReconnect()` take, and
the compensating `purgeServerRegistries` call iterates an already-emptied set,
so nothing else writes the tombstone there. A replacement connection
republishing a byte-identical declaration therefore replayed the
pre-reconnect review instead of asking for a fresh one. Mirror the sibling
site; keep the `DiscoveredMCPTool`/`serverName` filter and run before the
re-registering `await`.

R4-8: the `select:` dedupe key keyed unresolvable names on the canonicalized
alias, so two distinct requested spellings that canonicalize to one alias
(`task` and `agent`) collided on a single `seen` key. The second was dropped
before the `maxResults` check and landed in none of missing/truncated/
ambiguous, silently shrinking the model-facing report (`Not found: task` /
`1 missing` instead of `Not found: task, agent` / `2 missing`). Keep the
resolved-identity key — that half is the case-variant fix — and key only the
unresolved branch back on the requested spelling.

* refactor(core): drop the reviewed-declaration tombstone, gate on the record

The tombstone existed so the review map could be pruned on removal without
opening the never-reviewed pass-through. It did not hold: the record loop
fingerprinted only tools hidden at render time, so a tool reviewed while
revealed kept no entry, its removal wrote no tombstone, and a replacement
publishing a changed contract was invoked with arguments the model wrote
against the schema it actually saw.

Remove the pruning rather than complete it. A retained entry can only match
the same server, schema name and parameter schema, so it either still
describes the live tool or forces a re-review, and no removal route has to
remember to touch the map. Record every tool tool_search returns, not only
the hidden ones, so reveal state no longer decides gate coverage.

Answers round-5 R5-1 (ambiguous select: repeats had no witness) and R5-2
(the reveal-state hole); moots R4-1, R4-5 and R4-6, which were all
properties of the tombstone. Design doc updated in both languages.

active declaration list.
2. The model calls `tool_call` with the exact deferred tool name and arguments.
Both halves resolve a requested name the same way, so the schema `tool_search`
returns is the tool `tool_call` invokes: a legacy alias is canonicalized first
(`search_file_content` → `grep_search`, `replace` → `edit`, `task` → `agent`),
then an exact registered match wins, then a single case-insensitive match
resolves. A name that matches several registered tools only by case resolves to
none of them — registration order decides nothing — and both halves refuse it:
`tool_search` reports it as ambiguous and lists the spellings that do resolve,
`tool_call` refuses and asks for the exact name. A tool that was never reviewed
in this session still resolves by name.
When `tool_search` returns a tool, it records a fingerprint of that tool's
invocation contract — its MCP server (empty for a built-in), the name in its
schema, and its `parametersJsonSchema`. `tool_call` recomputes the fingerprint
and refuses a hidden tool whose live value differs, telling the model to re-run
`tool_search` with `select:<name>`. A re-review overwrites the record, so that
loop closes in one round trip. Recording is not gated on whether the tool is
hidden at review time: a revealed tool can be hidden again later, and gating
the record on reveal state would make the comparison's coverage depend on state
the model neither controls nor observes. The free-text `description` is not part
of the fingerprint: shipped deferred tools rebuild it from mutable state on
every `schema` access (`web_search` interpolates the current month, `read_file`
the effective input modalities), so hashing that prose would refuse calls whose
parameters still match the reviewed schema.
The existing deferred-tools startup reminder carries the compact live catalog

the wrapper bucket, and so does any refusal that resolves to no target — an
ambiguous name has no validated target to account against, so its strike lands
in the same `tool_call` bucket as a malformed envelope.
## Known limitations
`tool_call` routes a hidden deferred tool by name alone: it does not record a
per-session presentation mark when `tool_search` delivers a schema, and it does
not compare a captured schema fingerprint against the live schema at call time.
A model can therefore invoke a hidden tool whose schema never entered the
active model context (the startup reminder already lists every hidden deferred
tool's name and description), or whose schema changed after a mid-session MCP
re-discovery. V1 deliberately accepts this name-only authorization boundary.
The presentation-mark and fingerprint precondition proposed in #6721 is
deferred to #11321 rather than required by this bridge.
`tool_call` records no per-session presentation mark. A hidden tool whose schema
never entered the active model context is still invocable by name, because the
startup reminder already lists every hidden deferred tool's name and
description, and only a tool `tool_search` returned in this session carries a
fingerprint to compare against. The declaration check therefore narrows the
name-only authorization boundary instead of closing it; the presentation-mark
half of the precondition proposed in #6721 remains open in #11321.
A change limited to a tool's `description` is not detected, deliberately: those
getters must keep recomputing so a long-lived process is not stale across a
month boundary or a mid-session model switch.
The fingerprint's only server component is the server's name label, so a
replacement server republishing identical tool names and schemas is accepted
with arguments written against the previous connection. Telling a connection
identity apart from a name label needs a channel the registry does not have
today; it is tracked in the #11321 discussion rather than approximated here.
Review records live on the registry instance, survive `/clear`, and are never
pruned. An entry can only match the same server, schema name and parameter
schema, so a stale one either still describes the live tool or makes `tool_call`
ask for a fresh review. Pruning on removal would invert that: a dropped entry
reads as "never reviewed" and passes a replacement through. The map is bounded
by the distinct tool names reviewed in the process.
