feat(providers): add Command Code (#1658)
* feat: Command Code support

Reads the cmd CLI's session transcripts at
~/.commandcode/projects/<slug>/<id>.jsonl (CODEBURN_COMMANDCODE_DIR moves
the root). Checkpoint and prompt sidecars are skipped.

usage.inputTokens includes cacheReadTokens, so fresh input is
inputTokens - cacheReadTokens. The recorded costUsd is kept per call
(costFromBilling); a call without one is priced from tokens and marked
estimated. Forks copy parent message ids into a new file, so calls dedupe
by message id across sessions.

Desktop: Electron sandbox read path, macOS and Windows change watchers,
Windows tab colour. No quota adapter.

* fix(command-code): dedupe on meta.messageId, not the 8-hex line id

The top-level id is randomUUID().slice(0, 8) and only unique within one
file, so calls from unrelated sessions could collide and vanish.
meta.messageId is a full uuid that survives forks. Fixture ids now use
the real 8-hex shape.

* test(command-code): check sidecar skipping by file name, not a posix path

Restore usage from Mistral Vibe Unified Harness sessions (#1599)
* Restore usage from Mistral Vibe Unified Harness sessions

* fix(mistral-vibe): read unpinned Vibe 2.26 unified sessions exactly

An unpinned session has session_metadata.active_model null, so the parser
dropped it entirely. Fall back to config.toml's active_model, then Vibe's
default mistral-medium-3.5.

Split the rotated journal prefix into as many calls as runtime state lists
completions, so call counts match. Drop the tool group prefix from history
names (file_system.bash) and attach a turn's tools to its first call only.

Re-derive settled days (DAILY_CACHE_VERSION 58). Adds a redacted fixture from
a real 2.26.0 session.

fix(antigravity): count cache reads and price the standalone app's model (#1655)
* fix(antigravity): count cache reads and price the standalone app's model

The standalone Antigravity app (2.19) stores usage as ModelUsageStats in
gen_metadata. Field 5 (cache_read_tokens) was never read, on the SQLite
path or the RPC path, so every cached prompt token was dropped. Fields 9
and 10 are thinking and response output, the reverse of what the SQLite
path assumed. The rows carry only MODEL_PLACEHOLDER_M16 and
"gemini-pro-default", so the model priced at $0.

Read cache read (and cache write) in both paths, read 9/10 in the right
order, drop the input fallback to field 1 (the model enum), and map the
placeholders seen in the app's own model catalog, flagged as estimated.
Bump the antigravity results cache, its parse version and the daily cache.

* fix(antigravity): keep .pb history across the cache bump, step times, 200k tier

With the results cache at v8 and Antigravity closed, .pb cascades had no
entry and no server, so they vanished until the app was next opened.
Serve the v7 entry meanwhile, stored with mtimeMs -1, and preload the
results cache so the session cache reparses such sources and the next run
with the server up re-fetches them with cache reads.

Standalone .db rows without created_at take their first step's time
(steps.metadata #1), which equals created_at wherever both exist, instead
of the file mtime.

Price antigravity at the Gemini long-context tier: add it to
TIERED_PRICING_PROVIDERS and pass the provider at both calculateCost sites.

fix(copilot): count agent-host requests and their output (#1651)

* fix(copilot): count VS Code agent-host requests and their output

VS Code now runs Copilot's agent through the Copilot CLI engine. Its
assistant.message events carry no outputTokens (CLI 1.0.8x writes the same
shape), so every request was skipped and the shutdown rollup, which assumed
per-turn events own output, dropped it: 0 calls and 0 output.

Tokenless messages now count as calls with zero tokens, and a leg with no
per-turn output takes its output from the rollup. The rollup also carries
modelMetrics totalNanoAiu, so the session's credits are exact. Tokens and
credits appear when the session ends.

* test(copilot): serve an open agent-host session

* fix(copilot): serve store-session output via the residual, count credit requests

CLI 1.0.8x sessions with session-store rows lost their output: the rows
replace the rollup and carry output only for compaction requests. The
per-leg residual now carries rollup output minus compaction-row output.

The credits note counted store rows, rollups and per-turn twins as separate
requests. It now counts requests by behavioral weight, rated when the
session carries an exact figure.

* fix(copilot): keep paired store-row credits, running nanoAiu delta

A behavioral store row (compaction or crash tail) made the credit sum drop
every paired row. Store rows now always count; a rollup counts only when
nothing behavioral carries nanoAiu.

totalNanoAiu is a running bill that does not reset at compaction, so its
per-leg delta uses its own sentinel instead of the token reset.

copilot's PENDING_REDERIVE contract moves to 56: a store row and its
message straddling midnight can move one call to the next day.

Read DSH session format v4 written by dsh 0.2.0-rc.2 (#1612)
* Read DSH session format v4 written by dsh 0.2.0-rc.2

dsh 0.2.0-rc.2 stamps every session log v4, so discovery skipped each one
and DeepSeek Harness contributed no usage anywhere (#1584).

Admitting v4 only after verifying the reader's whole consumption surface
against DSH's official sessionFormatCatalog (recovery: 'strict',
validation: 'current', @deepseek-ai/dsh-session-format-catalog@0.2.0-rc.2),
per the semantic-update rule docs/providers/dsh.md prescribes for format
bumps: dense seq, usage at assistant/message data.usage (plus a new
informational totalTokens sum) or the embedded stream, tagged end-seed
inheritance, and llm/retry-started attempt slots are all unchanged. What
v4 does change (tool-result role lift, plugin: tag namespacing, surfaceOp
append/replace metadata, new header fields, untagged end-seed markers in
unseeded sessions) is ignored or already handled.

Adds catalog-validated v4 fixtures (same buckets as v0-v3 plus the
informational totalTokens) and extends the per-version parse tests to v4.
Bumps the dsh parse version so cached skip-era sessions recompute.

Verified end to end against a real 0.2.0-rc.2 log: doctor reports the
session readable, and the parsed totals (12 calls, input 11783, output
3699, cache read 107520) match the official catalog artifact exactly.

* daily cache v52; drop unused field

fix(cache-lock): coalesce heartbeat ticks; make time-sensitive tests deterministic (#1559)
* fix(cache-lock): coalesce heartbeat ticks so the fence never waits behind a backlog

Each interval tick queued a full heartbeat in the owner serializer, so an
interval firing faster than a tick completes grew the queue without bound and
verifyStillOwner waited behind all of it. The 1ms-heartbeat fence test hit its
30s timeout on a loaded runner and retried up to 10 times, which is the silent
cache-lock suite hang in CI.

* test(menubar): hold off the helper's SIGTERM until the stand-in app records its status

The relaunch helper signals the app as soon as the CLI child is reaped, which
could land between the stand-in's fopen and fclose and leave an empty status
file. Also wait for the recorder's full line instead of a fixed sleep.

* test: pin day-boundary tests so they pass at any time of day

CLI children that query "today" now run in a fixed-offset zone where it is
about noon on the UTC date (tests/fixtures/noon-tz.ts), so fixtures a little in
the past are always today; the old clamp to UTC midnight put them in the future
for the first minutes of the day and zeroed the counts. The month-scoped budget
and project-filter fixtures get the same treatment (the latter also failed on
days 1-5 of every month), the cursor lookback case derives its start from now
instead of a date that ages out on 1 Oct, and the renderer tests that format a
stamp from a few hours back pin the clock to mid-day. The session drawer date
is formatted in the local zone, so its expectation is too.

* test: derive dated fixtures from the clock so rolling windows never age them out

The six-month --period all window and the sync ledger's six-month prune
dropped fixtures fixed at April-July 2026 from 1 Nov 2026 on. CLI fixtures now
sit 30 days back (utcDaysAgo), the ledger and push tests pin Date to July 2026,
the --to-only range uses yesterday, and the date-picker tests pin mid-month so
two days back is on the calendar they open (they failed on the 1st and 2nd of
every month). The noon-tz helper moves to tests/fixtures/clock.ts.

* test(desktop): give spawned stand-ins room to boot before the silence window

The watchdog and graceful-kill tests used 1-1.5s silence windows, and the
window before a child's first byte includes node's boot. On a loaded machine
the child was signalled before it installed its handler or wrote its byte.
The windows are now 3s and the last-byte test measures from the byte the child
stamps. The killAll and fallback-scheduler tests wait on their condition with
a 15s deadline instead of a 2-3s one.

perf: store the session cache in day pieces (#1550)
* Store the session cache in day pieces with a per-provider index

Layout v10: a provider's files live in pieces by the UTC day of their
newest call, each with a key file, and an index row per file (piece,
fingerprint, call span, v9 month) read on every load. A ranged load reads
the index, the key files and only the members it reports on; a save
rewrites only the pieces it touched. Rows keep v9's full-load order and
month scope, so orphans, the first-paint snapshot and dedup see what they
saw on v9. The v9 month shards re-lay out once, streaming, and are removed
after v10 publishes.

* Skip cached transcripts outside a narrow query's months when unchanged

v9 could not see them, so every narrow query re-parsed each older Claude
transcript it discovered and republished the cache. The index knows their
fingerprint; an unchanged one needs nothing, a changed one still re-parses
as if uncached.

* Read piece members in batches through one buffer, and full loads concurrently

* Write piece key files without a sync; rebuild lost keys from the piece

* Changelog: day pieces

* Seed dedup with the keys of unchanged transcripts outside the query's months before a whole-file parse

* Upgrade-path check: verify the v10 envelope, indexes, pieces and key files

fix(menubar): older menubar apps load again, and in-app Update restarts the app (#1548)
* fix(menubar): always emit avgCostPerSession for older menubar apps

Menubar 0.9.14-0.9.24 decode topProjects[].avgCostPerSession as required,
so omitting it for partial session counts broke Today with any 0.9.25 CLI.
Emit the 0.9.24 value again and keep sessionCountBasis so 0.9.25+ hides
a partial average. Add a test that checks the payload against the
required keys of every released menubar app.

* fix(menubar): restart the app after an in-app update

The app runs `codeburn menubar --force` as its child, and pgrep/pkill
skip their own ancestors, so the running app was never stopped and
`open` re-activated the old copy. Find the app with pgrep -a; when it is
an ancestor, swap the bundle while it runs, exit, and let a detached
helper stop it and open the new bundle. The terminal path stops the app
first as before.

720824e619 fix(cli): default NODE_ENV to production so Ink's React build stops leaking user-timing entries.
React, loaded by Ink, picks its development build unless NODE_ENV is
exactly "production". Since React 19.2 that build records a
performance.measure() entry on every render (its DevTools "Scheduler"
and "Components" tracks), and Node keeps user-timing marks and measures
in a process-wide buffer that nothing ever trims. The interactive
dashboard re-renders on every progress tick and refresh, so the buffer
grows by roughly 700 entries a minute; after a few days the heap hits
V8's 4 GB default and the process aborts with "Ineffective mark-compacts
near heap limit ... JavaScript heap out of memory".

The launcher (dist/cli.js, the entry every client spawns) now defaults
NODE_ENV to "production" before importing main.js, which selects the
build that records nothing; an explicit NODE_ENV still wins. For that
explicit case, a small guard drops the user-timing buffer every 30 s for
the lifetime of each Ink UI (dashboard, sessions, context, compare).
codeburn records no marks or measures of its own, so nothing is lost.

Tests spawn the real launcher against a stub main.js to pin the default,
and drive a ticking Ink app in a child Node process to show the
development build's backlog, the production build's absence of one, and
the guard keeping it bounded.

4eabda17d7 fix: bound the serve drain, reap orphans on Windows, and de-flake the grace test
G1a - the stdin-EOF drain is now bounded (Promise.race against 45s, overridable
via CODEBURN_SERVE_DRAIN_MS for the test). Generous enough that no legitimate
request answered at EOF comes near it, and an async-wedged one releases the
child instead of turning it into the orphan the drain was added to prevent. The
comment names the ceiling: a SYNCHRONOUSLY wedged parse never yields to this
timer or to any other JS path, and only a signal can end that process.

G1b - Windows is the majority of the desktop fleet and had no orphan escape at
all: no ps, and no stdin-close recourse once the app has crashed. The reap now
works there. tasklist cannot report argv (image name and window title only) and
wmic is gone from current Windows, so the command line comes from
Get-CimInstance Win32_Process. Match discipline is unchanged - exact argv - with
quoting normalized on both sides, because we record a plain space-joined argv
while Windows reports the real quoted command line; a path with spaces still
round-trips. serveCommandMatches is extracted and tested directly.

G2 - the grace test raced node's cold boot (a 300ms window). The child now emits
one byte the moment it is ready, so the watchdog window is measured from
READINESS, not from spawn: boot cost cannot eat it at any machine speed. Timeout
raised on top of that. 6/6 green under 8 busy loops.

G3 - a served request heartbeats for its whole duration now, not just its parse:
the wrap moved to the one seam (runCaptured) that also covers aggregation and
payload serialization, whose ~8s tail lands back-to-back with the parse's own
quiet stretches. The one-shot path keeps the parse-only wrap, where an 8s tail
is comfortably inside the window on its own, and the CHANGELOG now says exactly
that instead of claiming every phase.

G4 - stillCold() anchors to coldStartBegan ?? bootedAt.

Text: the cold-timeout bound no longer claims to restore the "locate the CLI"
recovery (that is a not-found state, never cold-flagged), and the CHANGELOG
states the >15min case honestly - the ceiling still ends it, but partial cache
saves mean successive polls converge rather than each rescanning from nothing.

4fa16a2293 fix(dsh): correct usage attribution against the real session format
Reviewed src/providers/dsh.ts against deepseek-harness @ 99f6f02f and fixed
what the format says but the parser did not:

- A forked session's log replays its parent's events verbatim, and codeburn
parses the parent's own log as its own session, so every inherited call was
  billed twice. The header's parentSession + seedLength mark that prefix;
events with seq < seedLength are now skipped.
- The model now comes from the reporting assistant/message's own
message.source, which is what actually served the step. request/header only
describes the request DSH was about to make, and is the fallback.
- user/message also carries agent-injected context (runtime snapshots, skill
bodies) under source.kind 'plugin'; only a typed prompt becomes the preview,
and it is bounded to 500 chars like every other provider rather than holding
a whole injected system prompt per turn.
- A log stamped with a session format version other than 0 is skipped with a
notice. The format is pinned at 0 upstream with no compatibility implied, so
reading a bumped format under today's assumptions would report confident
  wrong numbers.
- Timestamps go through the seconds-vs-milliseconds guard and fall back to the
header createdAt, so a call can no longer carry an empty timestamp and land
  in the undated cache shard.
- The compressed read buffers the whole log to scan its frames, so it now takes
the same oversize guard readSessionFile applies to the uncompressed variant.
- The zstd-unavailable notice fired once per session log; each distinct notice
  is now emitted once.
- Emit workingDirectory beside projectPath, as codex does.

Tests add the upstream examples/acp-agent snapshot as a fixture, covering the
real record shapes: packed reasoning-chunks/tool-call-chunks storage rows, a
plugin-injected user/message beside the typed one, and both the streamed usage
chunk and the final assistant/message usage for the same step. Plus the same
snapshot re-encoded as multi-frame zstd with a torn tail (identical output), a
forked session, an unsupported format version, and unparsable lines.

0bfdfa372a perf(cache): shard the session cache per provider
A warm launch rewrote the entire session cache whenever any provider
appended a few KB: on a 6 GB corpus that is a 155 MB stringify + fsync
every run. The on-disk cache is now a version-suffixed directory holding
one shard per provider plus a small envelope, and a save rewrites only
the providers marked dirty.

- Dirtiness is tracked per provider (markCacheDirty) instead of one
global flag, so an appended Claude session no longer republishes
  Codex, Copilot and the rest.
- Shards carry a nonce in their filename and the envelope is renamed
last, so a save is published at a single point: readers never see a
half-updated set, and a writer that loses the refresh ownership fence
  leaves the canonical shards untouched.
- A shard that fails validation is treated as an absent provider rather
than rejecting the whole cache, so one malformed turn costs one
  provider's re-parse instead of every provider's history.
- v7 migrates losslessly: the blob is re-laid-out into shards and
removed only once that save publishes. Nothing re-parses.
- Cold-parse progress saves now trigger every N files parsed rather than
every 5s, so a slow cold parse no longer rewrites the growing cache on
  a wall clock.

ccee28ae82 fix(sync): address attribution review — cwd-fallback egress, Windows paths, PR-link validation
Review findings on the --attribution PR:

- Privacy: sessions whose project path no longer resolves inherited the
cwd-fallback repo identity, egressing whatever (possibly confidential)
repo the user pushes from and falsely attributing its commits.
buildRepoGroups now tracks per-session identity provenance; the
attribution path excludes fallback sessions from commit attribution
entirely (no repo, no commits, PR links only) — they also can no
longer steal a commit from a genuine session's window.
- Privacy: Windows drive-letter paths (C:/..., C:\..., drive-relative)
parsed as scp-like remotes, emitting local filesystem paths as repo
identities. normalizeRemoteUrl rejects drive letters and
single-character hosts (dotless intranet hosts still accepted).
- Hardening: PR links are shape-checked before sending (https,
/org/repo/pull/N path, <=256 chars, max 20 per session) — upstream
  parsers only truthiness-check them.
- Safety valve: MAX_ATTRIBUTION_PER_PUSH (10k) caps a first
--since all --attribution push; dry-run reports the cap.
- Tests: adversarial normalize corpus, cwd-fallback egress repro,
  commit-stealing prevention, PR-link sanitization, and CLI-level tests
(mock IdP + collector): dry-run sends nothing to the traces endpoint,
flag-off emits no attribution span names on the wire.
- Docs: reconciled the 'never sent' wording with reality (PR links ride
even when repo is null; device_id/methodology/timestamps disclosed).
  CHANGELOG Unreleased entry added.

d5144593f3 fix(cache): recover a corrupt session-refresh lock instead of freezing ingestion
observe() classified a stable unparseable session-refresh.lock body as the
terminal 'unavailable'. parseAllSessions routes that to a read-only parse, so a
zero-byte or truncated lock froze warm-cache ingestion permanently across every
later run while each command still exited successfully.

A corrupt body is now a recoverable observation carrying a real mtime, and is
recovered only through the UNMODIFIED staleness gate — tryTakeover and the age
check are byte-identical to main. sameObservation gains an explicit null/non-null
boundary and a sha1 of the raw bytes, because two corrupt bodies have no tokens
to compare and mtime granularity is coarse on some filesystems.

The heartbeat deliberately does NOT rewrite a body it cannot prove is its own.
An owner that cannot prove ownership ends its ownership: mtime stops advancing,
the publication fence refuses, and a successor recovers the lock one staleMs
later. Losing that parse is the price of never having two owners.

cache: strict cross-process gate for the warm session-cache refresh transaction (#743)
* cache: strict cross-process gate for the warm session-cache refresh transaction (#645)

* lock: serialize the fence against the owner's own heartbeat

verifyStillOwner and the heartbeat tick both take the takeover guard;
without in-process serialization the fence could observe its own
heartbeat's guard file, read it as displacement, and abort a legitimate
publication. Fail-safe, but it discarded the parse the lock exists to
protect (~6% of verifies at a 1ms heartbeat in the repro). Owner-side
guard operations now run through one serializer; cross-process guard
semantics are unchanged. Regression test at heartbeatMs 1, mutation-
verified against the unserialized code.

d8a7b2a95f fix(sync): address PR review — https enforcement, keychain test isolation, golden ID pins
Before-merge items:
1. HTTPS enforced on every remote endpoint (RFC 8252 §8.3): baseUrl,
issuer, authorization/token/revocation endpoints, and the traces
endpoint all reject non-https, with a loopback (127.0.0.1/::1/
localhost) exception for offline tests and local dev. Enforcement is
central (assertHttps) — the browser-open guard is no longer the only
   check whose failure was swallowed.
2. Credential store test isolation: CODEBURN_SYNC_TOKEN_STORE=file
forces the file store (honors HOME) so the offline suite never
touches the real macOS login keychain. Set in the e2e suite.
3. Golden pins for deriveSpanId/deriveTraceId/deriveDeviceId with fixed
inputs and expected hex — the idempotency contract depends on these
encodings being stable across releases; a green-tests encoding change
would silently double-count history on span-ID-keyed backends.
getDeviceId refactored over a pure deriveDeviceId(host, user).

Smaller review items:
- Callback server: ready promise resolves the actually-bound port from
  the listening event (kills the 100ms-sleep race after port fallback);
Connection: close on all responses + closeAllConnections() on
shutdown (pooled keep-alive sockets from a closed server could
swallow requests aimed at a later server on the same port); error
handler guarded so a post-bind error can never rebind to a different
port than advertised; optional ports param ([0] = ephemeral) removes
fixed-port contention between parallel test workers.
- fetchOidcConfig verifies the issuer claim matches the fetch origin
  (OIDC Discovery §4.3 mix-up defense).
- partialSuccess.rejectedSpans wrapped in Number() — proto3 int64 JSON
mapping sends strings from strict protojson servers; += would
  concatenate.
- Ledger writes are atomic (temp + rename); corrupt-ledger recovery and
no-tmp-left-behind tests added; XDG_CACHE_HOME honored (ledger is
  reconstructible state, not config).
- Mock IdP now implements /oauth2/authorize (registers PKCE challenge,
  302s to redirect_uri) and verifies S256 code_verifier + single-use
codes at the token endpoint. The e2e drives the real redirect flow
and asserts wrong-verifier and code-reuse are rejected — PKCE binding
is now exercised end to end.
- sync reset calls clearLedger() instead of reimplementing the path.
- push sets exit code 1 on rate-limited/server-error outcomes so cron
and script callers can detect incomplete pushes.

Deferred (noted for fast-follow): macOS 'security -i' stdin mode
(untestable on this Linux box), ai.cost_estimated as a real
ParsedApiCall flag (touches core parser types).

Sync suite: 81 passing (5x stable), 5 developer-only.

163842edac test(sync): unit, mock-IdP e2e, and developer-only integration tests
Offline, CI-safe (70 tests):
- sync.test.ts (26): discovery parsing, PKCE, auth URL, scope
negotiation, callback server (state mismatch, IdP error, timeout,
  port fallback), config round-trip
- sync-ledger-otlp.test.ts (23): deterministic ID derivation, OTLP
  structure/attributes, batching, ledger append/dedup/prune/clear
- sync-push.test.ts (15): pipeline against a scriptable mock OTLP
  server — success+ledger+cost, partial-success batch NOT ledgered,
  401 stop, 429 wait-and-retry / persistent-429 give-up / Retry-After
  parsing (delta-seconds, HTTP-date, garbage), 5xx deferral,
  idempotent failure recovery
- sync-e2e.test.ts (6) + fixtures/mock-idp.ts: full setup→refresh→
  rotate→logout round-trip against an in-process mock IdP

Developer-only (skip unless env vars set, never CI):
- sync-headless-e2e.test.ts (1): real browser PKCE flow via Playwright
- sync-infra-e2e.test.ts (4): push/auth-reject/batch/idempotent
  re-push against a deployed backend

b95b62d8fa fix(cli): surface mDNS scan errors, robust spend-flow rollup + date label, harden tests
Addresses T1 review: discovery.ts logs mDNS bind errors to stderr and returns
already-found devices instead of masking failures as empty; spend-flow uses a
collision-proof "other" sentinel and a local-date period label; the spend/date
flags get friendly error handling; devices-json tests are deterministic (mocked
discovery) and assert the read-only mutation guards. 13 tests green, tsc clean.

Implemented by Codex gpt-5.5 (high); committed by Fable (git blocked in Codex sandbox).

e890d9bfc3 test(security): add failing test for HIGH-1 prototype pollution
Three PoC fixtures (tool name, bash command, model name) reproduce
the audit's HIGH-1 attack. Tests assert Object.prototype.calls stays
undefined after parsing. They fail against current parser.ts -- Task 3
will close the pollution sink with Object.create(null).
